For years, it’s been our honor to care for you and your family. Though this chapter is ending, your health story continues — and we’re here to ensure you have access to your records every step of the way.

Oasis Family Medicine has officially closed.
We are grateful for the opportunity to have served you.

📁 Requesting Your Medical Records

In compliance with California law, we will retain all patient records for at least seven (7) years from your last date of care.

To request a copy of your medical records, please send an email to:

Your email should include:

- Full Name
- Date of Birth
- Your preferred email address for receiving the records
- *Picture, Screenshot, or PDF of your confirmation/receipt of payment from stripe for the records ($25)

🔐 How You’ll Receive Your Records

Once your request is processed, we will upload your records to a secure, password-protected OneDrive folder and email you the link.
The password will be sent in a separate message for security.

Please allow up to 15 business days for processing, in accordance with California Health & Safety Code §123110.

If your request is time-sensitive, please include “URGENT – RECORD REQUEST” in your subject line.

How to Request Superbills

In compliance with California law, we will retain all patient records for at least seven (7) years from your last date of care.

To request a superbill, please send an email to:

- Full Name
- Date of Birth
- Date of service/appointment
- *Picture, Screenshot, or PDF of your confirmation/receipt of payment from stripe for the records (*$15 per superbill)
